What really happens is that you send an SMS to an internet connected device, that device does the transaction, and then sends an SMS back.
The transaction itself is still done through the Internet.
You could do an off chain send without Internet. But if you want a transaction broadcast to the Bitcoin network you’re gonna need Internet somewhere along the way.

Can’t send bitcoin with it either. Can send a transaction request to unlock from a key and relock to another key, and a few similar options, scripts and such. No bitcoin is “sent.” No sats are “sent.” A signed request to update a ledger is sent.
